Visit
First we need to schedule an appointment. Please call 858-455-9391
to arrange a time for your initial evaluation. You will be asked a variety
of questions:
• What is your diagnosis?
• Did you recently have surgery?
• How soon would you like to come in?
• Who is your referring physician?
•
Is this an auto- or work-related injury? (Work-related injuries need to
have treatment authorized by the appropriate claims adjuster. This often
delays our ability to schedule your first appointment.)

Will we be billing
your private insurance?
What to bring on your first visit:
• Your prescription
from a physician
• Picture ID
• Insurance information card (PPO, HMO plan, or Medicare)
•
Change of clothes — shorts
and a loose-fitting T-shirt in most cases
PLEASE ARRIVE 15 minutes before your scheduled treatment time to complete
paperwork.
Insurance
If you are using your private insurance, you will be given a benefit
explanation sheet on your arrival. (We will have
received this information from your insurance company.) We strongly urge
you to contact your insurance company to verify this information. We can
only rely on the information they provide us, and occasionally they are
wrong.
Cancellation and no-show policy
Cancellations and no shows disrupt our entire office
procedure. We ask that you give us 24 hours notice if you need to reschedule
your appointment; otherwise your missed appointment is considered a
no-show. We strictly hold to one no-show per client — after that,
you are removed from our schedule. |
